Primary Care at Pagosa Springs Medical Center is designed to take care of your entire family. Our family practitioners deliver a range of acute, chronic and preventive medical care services. Primary Care is open Monday- Friday 7:30am – 5pm. We offer same day appointments for acute care and accept walk-ins on Saturday from 8am -4pm. We want our patients to live a healthy and active life, so in addition to diagnosing and treating illness, we provide recommended preventative care, checkups, immunization so you can maintain a healthy lifestyle.t.
We offer no appointment necesssary $26 flu shots and $25 Sports Physicals daily!
We accept Medicare, Medicaid, CHP+, Veterans Choice, commerical insurance, and we offer a sliding fee scale for under-insured and non-insured patients. To qualify for the sliding fee services, you will complete a brief application along with income documentation and a statement of the number of people residing in your home. Your first visit
Our Primary Care facility serves patients with a staff of multi-disciplinary healthcare professionals.
We ask that you show up 15 minutes prior to your appointment to allow time to get checked-in and any fill out any necessary paperwork.

With the Health-e Patient Portal you get easy, fast, and private access to your medical information and lab results online, so you can view your personal health record whenever and wherever you have access to the Internet. You can now also send and receive follow-up messages with your provider.
Another service provided by our Primary Care facility at Pagosa Springs Medical Center is the Early Detection Program. The program offers $39 cholesterol screens with counseling and follow up, as well as low cost self-ordered lab testing. With the focus on prevention, the program guides patients to develop a healthier lifestyle and/or seek medical services when needed.
