Benefits

In addition to a variety of employee perks, including primary care visit, medical center café, and phone plan discounts, Pagosa Springs Medical Center offers excellent health benefits to employees who work 30 hours or more per week.

Here is a summary (subject to change) of those benefits as of July 1, 2021.

Health Insurance

Provider: UMR
Effective Date: First day of month following employment
Who is eligible: All regular employees working 30 hours or more a week

Dental Insurance

Provider: Delta Dental PPO plus Premier plan
Effective Date: First day of month following employment
Who is eligible: All regular employees working 30 hours or more a week
Annual Deductible: Individual $25.00; Family $75.00
Annual Maximum: $1,500
Diagnostic and Preventive: 100% with PPO providers; 100% with Premier provider; 80% with Non-participating provider

 

Vision Insurance

Provider: Vision Service Plan (VSP)
Effective Date: First day of month following employment
Who is eligible: All regular employees working 30 hours or more a week
Well vision exam copay: $10
Eyeglass lenses copay: $30
Frames: Up to $130 allowance, then 20% discount on all exceeding allowance
Elective Contact lenses: $120 allowance
Necessary Contact lenses: Covered in full at VSP doctors
Laser Vision: 15% – 20% average discount or 5% promotional price

Retirement Program

Provider: Colorado Retirement Association (CRA)
Effective Date: Enrolled upon hire
Who is eligible: All employees
401(a): Employees are required to contribute 6% of their gross wage. Pagosa Springs Medical Center matches that amount on a five year vesting schedule.
457(b) Deferred Compensation: Employees may participate at any time.

Life Insurance

Provider: Lincoln Financial Group
Effective Date: First day of month following employment
Premium: Medical center pays 100% of premiums
Who is eligible: All regular employees working 30 hours or more a week
Benefit: 1x employee annual salary

Paid Time Off

Who is eligible: Hourly and salaried staff working at least 36 hours per week.
In addition to paid holidays, employees accrue personal float days and paid time off biweekly. Total annual accrual ranges from 21 to 32 days depending on years of service.

Flexible Spending Account/Health Savings Account

Effective Date: Immediate coverage if elected
Who is eligible: All regular employees working 30 hours or more a week
The Flexible Spending Account and HSA programs allow you to pay dependent day care costs, and out-of-pocket expenses for medical, vision, dental and other health care, with before-tax dollars.
