Physician/Provider Positions

Pagosa Springs Medical Center has an opening for a Part-Time, Non-Invasive Cardiologist.  Must be Board Certified/Board Eligible in Internal Medicine and Cardiovascular Disease.

  • Approximately 8 10-hour shifts per month.
  • Compensation: $2,000 per 10-hour shift.

Job Duties

This position provides integrated behavioral health care to Rural Health Clinic patients.  This type of care is delivered with a warm hand-off during the clinic day or is a scheduled visit. This position also provides typical social work supportive to patient needs. The IBHCP uses specialized education, discretion and judgement to perform the following: 1. Assess the patient’s physical, psychosocial, cultural and spiritual needs, utilizing evidence-based screening tools (PHQ9, GAD7, CSSRS, etc.); 2. Assist in the coordination of appropriate services, resources, and referrals; 3. Advocate and provide support to patients and families having difficulty coping effectively with changing medical conditions and other social determinants that may affect their health and well-being; 4. Diagnose and provide short-term evidence-based behavioral health therapy in the outpatient Rural Health Clinic; 5. Refer to a higher level of care when required for diagnosis and medication management; 6. Facilitate remote therapeutic sessions with offsite providers (Psychiatrists, Psychiatric NP, etc.) via telehealth when needed and act as a liaison between the specialist and the primary care provider. Duties may be changed as clinic needs are modified.

Job Requirements

    • Graduate of Accredited Program corresponding to licensure.
    • LCSW, LPC, or LMFT license required in Colorado.
    • Beneficial to have two (2) years social work experience in a hospital or similar Clinic setting; referral to community resources; discharge planning; and/or case management or other related experience.
    • Knowledge of community resources, hospital operations and excellent communication/presentation skills.
    • Ability to assess, DSM 5 diagnose and provide individual, family and group therapy.
    • Ability to perform suicide risk assessments and interventions to include out of area placements.
    • Prefer MAT training and implementation; substance abuse experience
    • BLS
    • Prefer experience with third party payment systems, i.e., Medicare/Medicaid.
    • Basic computer knowledge.

Pagosa Springs Medical Center has an opening for a Full-Time or Part-Time Primary Care Nurse Practitioner or Physician Assistant.  NPs must be certified by the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP). PAs must be certified by the National Commission for Certification of Physician Assistants. Full-time providers work four 10-hour shifts per week (not weekends). Part-time providers work a minimum of two 10-hour shifts per week (not weekends).

PSMC provides professional liability insurance coverage to all employed providers.  PSMC provides a CME allowance of $6,000 annually (gross and subject to taxes) for full-time providers and $3,000 annually for part-time providers. Full-time providers receive 260 hours of paid time off per year.

  • Compensation range for full-time Nurse Practitioners or Physician Assistants:  $119,000 to $136,000 per year.
  • Compensation range for part-time Nurse Practitioners or Physicians Assistants: $57.21 to $65.38 per hour.
