Primary Care

Primary care and family medicine at Pagosa Springs Medical Center provide primary care services to patients in all stages of life, including newborns, children, adolescents, adults, and seniors. Our dedicated primary care team consists of board-certified family medicine physicians, family nurse practitioners, and physician assistants, and a highly-trained support staff of registered nurses and medical assistants who work together to provide a wide range of acute, chronic and preventive medical care services for your whole family.
PSMC’s primary care clinic is open five days a week, and primary care appointments are available Monday through Friday from 7:30am to 5:00pm. Urgent walk-in visits are available Monday through Friday from 8:00am to 4:00pm for non-emergent acute illnesses or injuries. Conditions that require emergent care can be seen in the Emergency Department.
The Clinic is closed from 12pm-1pm.

Your first visit

Our Primary Care facility serves patients with a staff of multidisciplinary healthcare professionals.

 

We ask that you show up 15 minutes prior to your appointment to allow time to get checked-in and any fill out any necessary paperwork.

Patient Portal

With the Health-e Patient Portal you get easy, fast, and private access to your medical information and lab results online, so you can view your personal health record whenever and wherever you have access to the Internet. You can now also send and receive follow-up messages with your provider.

Early Detection

Another service provided by our Primary Care facility at Pagosa Springs Medical Center is the Early Detection Program.  The program offers $39 cholesterol screens with counseling and follow up, as well as low cost self-ordered lab testing. With the focus on prevention, the program guides patients to develop a healthier lifestyle and/or seek medical services when needed.
