Topics focused on PSMC’s new Care Team model to help patients develop a medical home, which includes their physician, nurse practitioner or physician assistant, a registered nurse, a medical assistant, and a patient service representative. Discussion also included the importance of preventative care and early detection, as well as the availability of specialists at PSMC.
